We’re looking for parents and carers of young people aged 12-25 to tell us about their experience of ReachOut services. The survey asks you to tell us a bit about you and your family, how you have used ReachOut and whether you found it helpful. The survey is confidential.

Who can participate?

We are interested in hearing from any parent or carer who has used ReachOut in the past 12 months, who cares for, or supports, at least one young person aged 12-25 years and who is living in Australia. It should take around 10 minutes.

At the end of the survey, you will also have the option to be entered into a prize draw to win one of 25 vouchers to the value of $100.
